1. Case Submissions & Accuracy

   • All case submissions must include complete and accurate information, including patient details, shade selection, and required specifications.

   • Incomplete or incorrect submissions may result in processing delays.​

2. Delivery & Liability

   • While we strive to provide high-quality restorations, Ceramic Arts Dental Lab is not responsible for delays caused by third-party shipping services.

   • The prescribing dentist assumes full responsibility for the accuracy of patient records, impressions, and case instructions.

3. Remakes & Adjustments

   • Remakes due to lab errors will be addressed at no additional cost.

   • Remakes resulting from incorrect impressions, prescriptions, or requested changes may be subject to additional charges.

4. Payment Terms

   • Payment is due as per agreed terms. Failure to comply may result in service suspension.

5. Case Modifications

   • Any modifications to an order after submission may result in additional fees.

   • Ceramic Arts Dental Lab reserves the right to decline or modify cases that do not meet quality standards.

6. HIPAA Compliance & Patient Data

   • We comply with HIPAA regulations and ensure the confidentiality of patient information.

   • By submitting this form, you confirm that necessary patient consent has been obtained for lab processing.
